General Practitioners' Knowledge and Clinical Practice in Management of People with Type 2 Diabetes in Iran; The Impact of Continuous Medical Education Programs.
CONCLUSION: The results of this study provide the evidence that the knowledge and clinical practice of Iranian GPs in management of type 2 diabetes were not satisfactory. Furthermore, traditional CME programs in diabetes management were not effective in changing the GPs' clinical practice. Barriers to the delivery of diabetes care in the Middle East and South Africa: a survey of 1,082 practising physicians in five countries
ConclusionsPhysicians in the Middle East and South Africa identified limitations relating to their patients as the main barrier to delivering care for diabetes, without giving high priority to issues relating to processes of care delivery. Further study would be needed to ascertain whether these findings reflect an unduly physician‐centred view of their practice. More effective provision of services relating to the prevention of complications and improved lifestyles may be needed.

Patient and prescriber characteristics among patients with type 2 diabetes mellitus continuing or discontinuing sulfonylureas following insulin initiation: data from a large commercial database.
CONCLUSIONS: About half of commercially-insured T2DM patients discontinued SU within one year after insulin initiation, and SU discontinuation was not strongly associated with a range of patient and provider characteristics. PMID: 29264933 [PubMed - as supplied by publisher]

Trends in antidiabetic medical treatment from 2005 to 2014 in Taiwan
ConclusionAntidiabetic treatment has become complex in Taiwan. Although combination therapy would become the major treatment strategy gradually, the underuse of insulin therapy must improve.

Comparison of Placebo Effect between Asian and Caucasian Type 2 Diabetic Patients: A Meta-Analysis.
Conclusions: The overall differences of the placebo effect on HbA1c changes as well as on body weight changes were not significant between Asian and Caucasian T2DM patients. The placebo effect on HbA1c changes and weight changes was not associated with baseline age, gender, baseline body mass index, baseline HbA1c, duration of diabetes, or study duration. Factors associated with the discontinuation of dipeptidyl peptidase-4 inhibitors (DPP-4is) after initiation of insulin.
Conclusions: A large proportion of patients discontinue DPP-4is in the real world when initiating insulin. The impact of physician characteristics in treatment decisions highlights the need for enhanced physician training and support as new clinical data emerges and therapy options are available. EE459 Early Intensification of Oral Semaglutide in the UK: A Cost-Effectiveness Analysis Versus Continuing Metformin and SGLT-2 Inhibitor Therapy
A number of people with type 2 diabetes experience clinical inertia, remaining in poor glycaemic control on oral antidiabetic medications such as metformin and sodium-glucose cotransporter-2 (SGLT-2) inhibitors rather than treatment-intensifying with a glucagon-like peptide-1 receptor agonist, despite an efficacious, orally administered option, oral semaglutide, being available. The present study evaluated the long-term cost-effectiveness of immediate oral semaglutide treatment versus continuing metformin plus SGLT-2 inhibitor therapy in the UK.
